{"id":341,"date":"2016-12-08T20:12:12","date_gmt":"2016-12-08T20:12:12","guid":{"rendered":"https:\/\/carebuilderspgh.com\/?p=341"},"modified":"2016-12-08T20:12:12","modified_gmt":"2016-12-08T20:12:12","slug":"health-care-providers","status":"publish","type":"post","link":"https:\/\/www.carebuildersathome.com\/pittsburgh\/blog\/health-care-providers\/","title":{"rendered":"HEALTH CARE PROVIDERS"},"content":{"rendered":"<h4><strong>Find Out What Seniors Think About Providers<\/strong><\/h4>\n<h5>Majority of Seniors Rely on Family or Have No One to Coordinate\u00a0Health Care Providers, New Survey Finds<\/h5>\n<p><strong>\u00a0<\/strong>According to a recently released national survey conducted by Harris Poll, 85\u00a0percent of seniors surveyed noted they have been diagnosed with a serious health condition,\u00a0and 64 percent have seen three or more health care providers over the past year for their care.\u00a0However, 34 percent reported a family member coordinates their medical care among all of the\u00a0health care providers they see, and 35 percent say no one does. This \u00a0highlights a significant gap in\u00a0care due to the multitude and complexity of chronic conditions affecting aging seniors.<\/p>\n<h5>2 in 3 Seniors Had No Help<\/h5>\n<p>More than 1,000 seniors were surveyed and it found that while a majority (61 percent) are\u00a0asked during all or most appointments if they understand their treatment plans, less than half\u00a0(43 percent) are asked about the treatments and medications they have been prescribed by\u00a0other doctors. And in the critical first few months following a hospital stay, nearly 2 in 3 seniors\u00a0who have ever been hospitalized (63 percent) report that no one helped coordinate their care.<\/p>\n<h5>Seniors Need Help Coordinating Care<\/h5>\n<p>Coordinating care across multiple providers who treat a single patient is a key strategy for\u00a0ensuring positive clinical outcomes, while also reducing costs. As the health care system\u00a0continues to evolve toward coordinated care models, there is also an increasing effort\u00a0to\u00a0offer seniors programs to help address needs beyond medical care \u2013 and the survey shows\u00a0seniors are interested.<\/p>\n<h5>Transportation Is Important<\/h5>\n<p>The survey found that 36 percent of seniors say they want more opportunities to engage with\u00a0people in their community. Almost one in three seniors (32 percent) say it is just as important for\u00a0care providers to offer transportation to medical appointments as it is to monitor blood\u00a0pressure. Other key survey findings include:<\/p>\n<ul>\n<li>Almost all seniors (95 percent) are satisfied with the quality of care they receive from\u00a0their health care provider.<\/li>\n<\/ul>\n<ul>\n<li>A slight majority of seniors (56 percent) want\u00a0their health care provider to offer specific programs to help manage their health (from a list that was provided).
