Provides personal hygiene services, housekeeping tasks and other related support services essential to the consumers health
\nObserves, records and reports all changes to supervisor
\nPerforms home management tasks including housekeeping, laundry, shopping and errands
\nPrepares and serves simple modified diets according to instruction and assists with feeding as necessary
\nAssists with bathing, dressing and grooming
\nAssists with toileting, including use of bedpan, commode or toilet
\nAssists self-directing consumers with transfers and ambulation including use of cane, walker, and wheelchair
\nAssists self-directing consumers with medication reminders or refilling the consumers prescription at the local pharmacy as specified on plan of care
\nProvides routine skin care. May assist self-directing consumers in applying nonprescription topical medications to skin surface
\nCollects routine specimens
\nObtains consumers weight
\nAssist self-directing consumers in reminding them of their home maintenance exercise programs
\nAssists the self-directing consumer with reminders on changing the tubing or refilling a tank in the use of their oxygen equipment<\/p>\n
Must have a phone or a way to communicate to the office daily
\nMust have a valid PA License
\nMust be able to follow the care plan written in the home
\nMust provide proof of
\no TB Test
\no Criminal Background Check and FBI when necessary
\no PPD documentation within one year of hire
