What Is the Medicare GUIDE Program?
The Medicare GUIDE Program (Guiding an Improved Dementia Experience) is a comprehensive care model created to improve quality of life for people living with dementia while easing the burden on family caregivers.
GUIDE provides coordinated support through an interdisciplinary care team and allows eligible Medicare beneficiaries to receive dementia-related services at home. This includes care navigation, medical oversight, and Medicare-paid respite home care services.
Who Qualifies for the Medicare GUIDE Dementia Program?
The Medicare GUIDE Program is available to individuals who:
- Are enrolled in traditional Medicare
- Have a diagnosis of dementia or Alzheimer’s disease, or are suspected to have dementia
- Live at home or in an independent living, assisted living, or memory care community (not a long-term care nursing facility)
- Are not enrolled in the Medicare hospice benefit (palliative care is allowed)
